Empowering Young People to Thrive Through Personalised, Therapeutic Support
At Life Skills Hub, we recognise that every child and young person is unique. That's why our Outreach Team delivers a highly personalised, therapeutic service designed around the individual needs, strengths, interests, and aspirations of each person we support.
Working with children and young people aged 4–25, our outreach provision goes beyond traditional support. We provide intensive, relationship-based interventions that help individuals develop the confidence, resilience, independence, and practical life skills needed to achieve their goals and reach their full potential.
What makes our service different is our commitment to creating meaningful connections.
We carefully match each young person with a dedicated keyworker whose personality, communication style, and experience best complement the individual's needs. This tailored matching process helps establish trust, engagement, and positive rapport from the outset, creating the foundation for successful outcomes.
Our experienced and highly trained keyworkers take a therapeutic and person-centred approach, working alongside young people at a pace that feels comfortable and achievable. We understand that progress looks different for everyone, and we adapt our support accordingly. By building strong, consistent relationships, we create a safe and supportive environment where young people feel understood, valued, and empowered to step outside their comfort zones.

Social and communication skills
Emotional wellbeing and resilience

Preparation for adulthood

Accessing education, training, employment, and community opportunities
Our School Inreach service is where a dedicated keyworker support within the young persons education setting, providing a range of wellbeing support which is tailored to the young persons needs, providing 1 to 1 or small group support for young person aged 4 to 25 years.
Our Intense Outreach service is where one of our dedicated keyworkers work with the young people with their home or out in the community on a 1 to 1 of 2 to 1 basis with young people aged 4 to 25 years old.
